Application Form:
Prescribed application forms can be obtained from the Notified Centers against DD drawn on the Nationalized Bank in the name of the "AMUPMDC" and payable at "Mumbai" for Rs. 500/- (Rupees Five Hundred Only).
Application forms will also be available from Association’s Office, AMUPMDC, Shrijee House, 75 Mint Road, Mumbai - 01 by post with an additional DD of Rs. 200/- for postal charges.
Forms will also be available from Selected Branches of Punjab National Bank (PNB) on cash payment of Rs. 500.00.
The Application Form must be filled by the candidate in his/her own handwriting using only Black Ballpoint Pen. The application form complete in all respect shall be submitted directly to the office of AMUPMDC, Shrijee House, 75 Mint Road, Mumbai 01, either in person or by Speed Post only No Private Courier will be accepted. The filled-in Application Form is to be duly returned in the Envelope provided by AMUPMDC with the following documents
Filled-in Application Form:
i. DD for Rs. 1,500/- (Rs One thousand Five Hundred only) drawn in favour of "AMUPMDC" and payable at Mumbai (for Asso-CET 2012 Examination Fee).
ii. Receipt-Cum-Identity Card
iii. Self Address Envelope with appropriate postal stamp (for sending back the receipt)
Application From should be submitted within the time in advance. AMUPMDC will not be responsible for postal delay.
Application Form Submitted by Speed POST must be accompanied by a selfaddressed and stamped envelope along with Receipt-cum-Identity Card for acknowledgement.
Incomplete application forms will be rejected.
The official at the AMUPMDC office while accepting the application form submitted in person shall issue the Receipt cum Identity Card attesting the photograph of the candidate with signature and stamp.

Note: Change of Examination Centre is not permitted.
Please Note: Association reserves the right to add or delete the examination centers.
Due to unforeseen circumstances during the conduct of Asso-CET 2012 at a Centre(s) like incidence of mass copying, leakage of question paper, entry of unauthorized person(s) in the Examination Centres/halls, natural calamities, riots, etc. or any other reason rendering it impossible for the smooth conduct of Asso-CET 2012 the Competent Authority may cancel the examination at such or all centre(s). It shall be further notified on the website regarding fresh programme for conduct of Asso-CET 2012 at such centre(s) only.
Syllabus:
Syllabus for the Asso-CET 2012 will be same as of HSC Examination of Maharashtra State Board for the academic year 2011-12 for Physics, Chemistry and Biology. Syllabus will be available on the Website of the Association.
Asso CET 2012 Examination Pattern:
The Asso-CET 2012 will consist of one common question paper in the subjects of Physics, Chemistry and Biology (Botany & Zoology) consisting of 200 Multiple Choice Question (MCQs) 50 in Physics, 50 in Chemistry & 100 in Biology. There is no choice among the questions. The candidate is expected to attempt all the 200 questions. Out of four circles provided for selection of answers against a question, the candidate should fill-up one circle only. Each correct answer will be given one mark. There shall be no negative marking for wrong answers. Scratching, overwriting, tick marks, erasers, whiteners & multiple answers will be considered as wrong answers and no marks will be awarded to them.

Subjects: Physics, Chemistry & Biology
Eligibility:
A candidate will be eligible for admission to Health Science Courses i.e. MBBS, BDS, BAMS, BPTh, BOTh, BSc-Nursing for the academic year 2012–13 on fulfillment of the following conditions:
The candidate may be an Indian National / NRI / Foreign National. The candidate is required to produce proof in support of his/her Nationality from a District Magistrate, Additional Magistrate or Metropolitan Magistrate. School Leaving certificate of HSC/12th Std indicating the nationality of the candidate as Indian or Valid Passport also shall be considered as sufficient proof of Nationality.
For Admissions to MBBS, BDS, B.Sc Nursing the candidate must be born on or before 31ST December 1995 to be eligible to appear for Asso-CET 2012. For admissions to BAMS candidate must be born on or before 1st October 1995. The Secondary School Certificate (i.e. SSC) or equivalent examination certificate or the certificates of age and nationality endorsing the date of birth will constitute the valid proof.
Eligibility Criteria for qualifying Examination for various courses:
For MBBS, BDS and BAMS:
A candidate belonging to open category must have obtained not less than 50% Marks in Physics, Chemistry & Biology subject taken together at the HSC (or Equivalent) Examination.
A Candidates belonging to Reserved Category must have obtained not less than 40% Marks Physics, Chemistry & Biology subject taken together at the HSC (or Equivalent) Examination.
For B. Sc (Nursing):
A candidate belonging to open category must have obtained not less than 45% Marks in Physics, Chemistry, Biology & English subject taken together at the HSC (or Equivalent) Examination.
A Candidates belonging to Reserved Category must have obtained not less than 40% Marks Physics, Chemistry, Biology & English subject taken together at the HSC (or Equivalent) Examination.
For BPTh, BOTh, BASLP rule no. 5.5.1, 5.5.2 and 5.5.3 is not applicable to this course, passing at Std. XII or HSC or equivalent examination is necessary.
